This form is required to be filled out for every student attending our driving class. Please download a copy and review our terms and conditions. You will need to submit a copy of this form in person.

Drivers Education Program

If you don't cancel your appointment within 24hours you will be charged a $45 fee - Classes are available every 2 weeks online from 6pm-9pm. After your two week period, you must schedule your behind the wheel hours.
